What are the documents that need to prepare for pick up the cheque of VAT refund?

Panwa: Please find the required documents for pick up the cheque of VAT refund as below:

the notification letter of VAT return (PP.72) with the signature of authorized person(s) and stamp company seal
if the authorized person(s) did not pick up the cheque by themselves, please prepare the additional document as following:
the power of attorney with the signature of grantor (s) (and company stamp), grantee and witnesses, and attached the duty stamp in amount of Baht 10.
the copy of affidavit with the signature of authorized person(s) and company stamp
the copy of Thai ID card or passport (if foreigner) for grantor (s) and grantee with signature
the original Thai ID card or passport (if foreigner) for grantor (s) and grantee to present to officer only.
